| 2. | Complete the Application & Information Form by hand or typewriter. You may also provide all the information requested in a separate document so you can do so using word processing software. |
| 3. | Print a copy of your current curriculum vita or professional resume. |
| 4. | Mail your Application & Information Form (including answers to the 7 questions), your vita or resume, and a check (or your credit card information) for $250 to:
When I receive your application fee and form, I will review your application promptly. Your application fee of $250 will be applied toward your registration if you are accepted. Upon acceptance, your application fee will be non-refundable although you could apply it toward another event in the future. If you are not accepted, your application fee will be returned (if paid by check) or refunded (if paid with a credit card). If you are paying with a purchase order (PO), your application will be reviewed once the check is received from your company, university, school district, or other organization. |
| 5. | Upon acceptance, you may pay the balance of the registration fee by credit card, check, or P.O. You will have several options when paying by credit card (one, two, three, or four payments). |
